Nothing can compare to the anxiety of bringing home a new, preemie baby. There is so much to consider, to do and to prepare for.
Our new Preemie Passport is a program designed to help make this journey from hospital to home a safe one.
Preemie Passport is Kidz Kare’s newest patient care program designed for premature newborns.
Most preemies have been in a neonatal intensive care unit with 24-hour nurse monitoring and treatments. Some will be on special medical equipment when they are discharged and many are on special formulas or medications. Many have also had physical therapy, speech therapy or occupational therapy. This can be overwhelming to a new mom or family.
Kidz Kare acts as the link between the hospital and the family to transition the infant safely to home. We meet the parents before discharge and coordinate with the primary nurse for specific home care needs and supplies.
After discharge, our nurses meet the patient at home to coordinate medication and feeding schedules, physician and specialist appointments, equipment and therapy needs, and answer many questions. We usually follow up every 2-3 days for assessments and weight checks and monitor growth and development.
Our nurses are a perfect resource for community services available to the family such as early intervention or mom support groups. One of the best aspects of the Preemie Passport is that a Registered Nurse is available 24 hours a day.
